The consumer [YOU] agrees to the following:  

 

1.  All phone correspondence between the Provider [Michele O’Mara, LLC] and the Consumer [You] will be treated with confidentially.

2.  The intent of phone correspondence is to offer the Consumer the opportunity to receive guided self-help for issues of concern by a Licensed Clinical Social Worker who isfunctioning in the role of coach, educator, and supportive guide (not as a counselor).  

3.  Phone counseling is solely directed by the Consumer.  Issues to be discussed are presented by the Consumer, with the Provider responding with text relevant to the issue presented by Consumer.  The Consumer is responsible for including or withholding information relevant to the concerns presented.  However, feedback and responses are most effective when all of the relevant information about the Consumer’s concerns are presented.

4.  Feedback provided by the Provider is a to be viewed as a helpful resource.  Any suggestions, advice, or direction that may be inferred from responses by Provider are simply tools to be used as the Consumer deems appropriate much like one would treat information in a self-help book. 

5.  Phone counseling is a flexible, personalized and innovative way to address concerns and pursue personal growth. The goals of in-person counseling and phone counseling are similar, yet it is  inconclusive how effective phone counseling actually is.  The Provider does, however, intend to offer the Consumer with the most helpful, constructively challenging, and supportive feedback possible, with the intention of positively impacting the life and well-being of the Consumer. 

6.   Both parties are free to terminate the phone counseling relationship at any time, for any reason.  Any pre-purchased exchanges are good for one year following the date of purchase.  In the unlikely event that the Provider determines it is in the Consumer’s best interest to cease the phone counseling relationship, the Provider agrees to refund prepaid fees accordingly.

7.  All prices are based on per-minute charge.  Phone sessions are not to be greater than or less than the  will be limited to the number of minutes purchased.  

8.  In the event of a no-show for a phone counseling appointment, the consumer forfeits his prepaid fees as a result of the missed session.

The Consumer has read and comprehended the above stated agreements.  It is understood by the Consumer that phone counseling with Michele O'Mara, LCSW is not mental health treatment.  Consumer is aware that the Provider is based in Indianapolis , Indiana and is governed by the law accordingly.  Consumer agrees that phone counseling is best utilized for life stressors that do not pose any risk to the Consumer’s safety or well-being.  Specifically, the Consumer agrees that major mental health concerns such as:  severe anxiety, major depression, substance abuse, chronic mental illness, or other mental health conditions that are not to be addressed by this service with Michele O'Mara.  These issues warrant treatment and diagnosis and are best addressed in a traditional counseling setting.

In the event in-person psychotherapy is needed, it is the Consumer’s responsibility to seek necessary care.  In the event you are currently in counseling with a therapist, it is advisable that you discuss your participation in phone counseling with him or her.  

I verify that I am not currently experiencing major mental health concerns; that I am not actively suicidal nor have I been within the last year; and I understand the limitations of phone counseling with Michele O'Mara as described above.
